The American health care system is falling short. While it serves some people well, many are left behind. Patients feel disconnected and unheard, with their needs overlooked in a system challenged by rising costs and persistent gaps in care. Older adults face even greater barriers to effective and coordinated care. To re-design the system, we must eliminate what does not work and scale what does—so that optimal health care is the standard for many, not an exception for a few. The models of health care delivery that we develop, translate into everyday practice, and scale for broad adoption aim to identify what will ultimately work better for more people. The work we do to achieve this is guided by a commitment to the following goals: • Prioritizing whole-person care that emphasizes disease prevention, management, and the essential role of primary care. • Realizing the unique needs of today’s older adults and the rapidly aging population of tomorrow • Improving early intervention in chronic disease diagnosis and management • Leveraging new technology to reduce administrative burdens and improve clinical decision-making • Aligning health care spending with services best matched to patients’ needs with the ultimate goal to lower overall health care costs

Transforming Health Care Delivery
The Duke-Margolis / West Health Care Accelerator is dedicated to transforming health care delivery to better serve the needs of patients, especially older adults. Our approach focuses on prioritizing whole-person care, understanding the unique needs of older adults, improving early intervention in chronic disease diagnosis and management, leveraging new technology, and aligning health care spending with services best matched to patients’ needs with the ultimate goal to lower overall health care costs.
